Forward Head Posture and Its Impact on Tissue Mechanic Sensitivity & Psychological Health

Abstract

Forward head posture (FHP) is a typical postural deviation which is increasingly prevalent in contemporary populations and is accompanied by serious musculoskeletal and psychological implications. This literature review focuses on the biomechanical nature of FHP and its connection with the tissue mechanosensitivity, chronic neck pain, and psychological health. It has been shown that changes in cervical position enhance mechanical loading, which causes muscle imbalance, peripheral sensitization, central sensitization, and low pain thresholds. Other psychological factors like stress, anxiety and fear-avoidance are also players in the process of interacting with the pain mechanisms to produce a biopsychosocial cycle. Knowledge of these interactions helps to justify the importance of thorough evaluation and multimodal/biopsychosocial rehabilitation interventions in order to achieve better clinical outcomes.
